Individualized Supported Living (ISL)
Individuals who receive services through Individualized Supported Living (ISL) live in a community home with 24-hour support and oversight provided by trained staff from our agency. While independence is always encouraged, staff members are available as needed to support individuals with meeting their personal goals, completing daily living tasks, health management and community integration. Examples of support include meal prep and cooking, exercise activities, home maintenance, money management, budgeting, scheduling appointments, shopping and community integration.
WCHS currently has three ISL homes in our community, with plans to develop more. Each ISL home is well maintained and houses 3 individuals who have been assessed to be compatible to live together. Homes are fully accommodated to meet the specific needs of each resident. Rent and expenses are shared among the three living in the home.
WCHS provides all medical supports, including medication administration, annual exams, transportation to and from medical appointments and any follow up appointments required. Transportation is also provided for all community access needs.
